Police Consent to Disclosure Statement
Applicant’s Consent to Conduct a Police Record Check or Police Vulnerable Sector Check- I consent to a search being made in the Winnipeg Police Service local database, Canadian Police Information Centre, and the RCMP National Repository of Criminal Records to find out if I have any of the following: convictions and sentences or pending charges pursuant to federal legislation including the Criminal Code, Controlled Drugs and Substances Act, Cannabis Act, Immigration and Refugee Protection Act (and its predecessor), Youth Criminal Justice Act (and its predecessors); where confirmed and authorized for release by the contributing police agency outstanding judicial orders pursuant to federal legislation including the Criminal Code, Controlled Drugs and Substances Act, Cannabis Act, Immigration and Refugee Protection Act (and its predecessor), Youth Criminal Justice Act (and its predecessors), including judicial release orders, recognizances, warrants, peace bonds, prohibition orders, absolute discharges for one year from the date of discharge and conditional discharges for three years since the date of order of discharge on the conditions in the probation order; conditional discharges imposed pursuant to a verdict of not criminally responsible on account of mental disorder for the period in which the conditional discharge is in effect; prevention and protection orders pursuant to The Domestic Violence and Stalking Act (or similar legislation from another province); protection orders pursuant to The Child Sexual Exploitation and Human Trafficking Act (or similar legislation from another province);
- I consent to the disclosure of the results of the above noted search (item 1) to me. I understand that it is my decision if I choose to disclose the results to a third party and that I am solely responsible for this decision. I consent to the Winnipeg Police Service verifying the results of my Police Record Check/’Police Vulnerable Sector Check if a third party inquires.
- I consent to providing my fingerprints to the Winnipeg Police Service to confirm that my identity is a match to a record for the purpose of completing my Police Record Check or Police Vulnerable Sector Check.
- I have reviewed and consent to the process in the Winnipeg Police Service Record Check/Police Vulnerable Sector Check Policy.
I consent to a search being made in the automated criminal records retrieval system maintained by the Royal Canadian Mounted Police to find out if I have been convicted of, and been granted a record suspension for any of the sexual offences that are listed in the schedule to the Criminal Records Act. I understand that, as a result of giving this consent, if I am suspected of being the person named in a criminal record for one of the sexual offences listed in the schedule to the Criminal Records Act in respect of which a record suspension was granted or issued, that record may be provided by the Commissioner of the Royal Canadian Mounted Police to the Minister of Public Safety and Emergency Preparedness, who may then disclose all or part of the information contained in that record to the Winnipeg Police Service. The Winnipeg Police Service will then disclose that information to me on RCMP C-480 Document and Certified Criminal Record Check. If I further consent in writing to disclosure of that information to the person or organization referred to above that requested the verification, that information will be disclosed to that person or organization.
